Where do I find the scan report attestation of scan compliance document template?

Please download the attached document and submit the "ASV Scan Report Attestation of Scan Compliance" to the PCI DSS (Security Standards Council).

This report should be signed and delivered together with the following reports that are available from Security Center:

• ASV Scan Report Vulnerability Details
